ДИНАМИКА ОСУЩЕСТВЛЕНИЯ ЛАБОРАТОРНОЙ ДИАГНОСТИКИ ДИФТЕРИЙНОЙ ИНФЕКЦИИ В РОССИИ

Аннотация

Проанализирована динамика проведения обследований на дифтерийную инфекцию в различные периоды эпидемического процесса дифтерийной инфекции в течение 32 лет. Несмотря на введение с 2023 года нового нормативного документа по лабораторной диагностике дифтерии, окончательный диагноз дифтерийной инфекции по-прежнему ставится на основании результатов бактериологического исследования. За весь период наблюдения наиболее низкие показатели по количеству обследованных лиц были в период ограничений по коронавирусной инфекции, с 2023 года отмечается увеличение количества проведенных исследований на дифтерийную инфекцию до уровня 2018 года, отмечается положительная динамика по увеличению количества обследованных лиц с диагностической целью, в том числе с диагностической целью при обследовании пациентов с ларинготрахеитом, ларингитом (до 98,4–98,6 % в 2023–2024 гг.), крупом, ретрофарингеальным и перитонзиллярным абсцессами (до 86 % в 2024 г.), инфекционным мононуклеозом (до 96,1 % в 2023–2024 годах). Отмечается уменьшение количества обследованных лиц с острым тонзиллитом и фарингитом до 95,5 % в 2024 году. С 2021 года токсигенные штаммы C. diphtheriae на территории страны не выделялись. В период высокой заболеваемости выделение нетоксигенных штаммов соответствовало характеру распространения токсигенных штаммов. Самые низкие показатели выделяемости нетоксигенных штаммов отмечены с 2014 по 2022 год, в 2024 г. был на уровне 0,56 и отмечается рост выделяемости до 1,06 за 6 месяцев 2025 г. до уровня 2010–2011 годов.

Annotation

The dynamics of examinations for diphtheria infection during various periods of the epidemic process of diphtheria infection over 32 years was analyzed. Despite the introduction of a new regulatory document on the laboratory diagnosis of diphtheria from 2023, the final diagnosis of diphtheria infection is still made based on the results of bacteriological research. Over the entire observation period, the lowest rates in terms of the number of examined persons were during the period of restrictions on coronavirus infection; since 2023, there has been an increase in the number of studies conducted for diphtheria infection to the level of 2018, there has been a positive trend in the increase in the number of examined persons for diagnostic purposes, including for diagnostic purposes when examining patients with laryngotracheitis, laryngitis (up to 98.4–98.6 % in 2023–2024), retropharyngeal and peritonsillar abscesses (up to 86 % in 2024), infectious mononucleosis (up to 96.1 % in 2023–2024). However, there is a decrease in the number of examined persons with acute tonsillitis and pharyngitis to 95.5 % in 2024. Since 2021, toxigenic strains of C. diphtheriae have not been isolated in the country. During the period of high morbidity, the isolation of non-toxigenic strains corresponded to the pattern of distribution of toxigenic strains. The lowest rates of isolation of non-oxygenic strains were from 2014 to 2022, in 2024 it was at the level of 0.56 and there was an increase in isolation to 1.06 in 6 months 2025 to the level of 2010-2011.
